Do you have a common health concern but don’t have or can’t get into your family doctor?

Common episodic concerns include eye, ear, sinus, throat, urinary, skin, minor wound, prescription refill, or medication injections.

During our wellness visits we emphasize well-being, illness prevention, and health promotion. The NP can diagnose and treat common illnesses and injuries, as well as order and interpret necessary diagnostic tests such as X-rays, ultrasounds, CT scans, MRIs, and bloodwork; assist clients in managing chronic health conditions such as diabetes, high blood pressure, or breathing issues; prescribe medications; provide health education and counselling; refer clients to specialists; and assist clients in navigating our complex healthcare system through multidisciplinary referrals.

The assessment fee is $200 and includes an in-depth physical exam, urine test, and antibiotic or medication prescription (if required).

In Ontario, all drivers must meet basic medical standards to operate a motor vehicle. For those individuals applying for a class A, B, C, D, E, or F licence in Ontario, you need to submit a medical report. This medical report can be completed by a physician or nurse practitioner (RN-EC) licensed to practice medicine in Canada and involves an in-depth assessment and completion of the medical report. The medical report form can be downloaded below or will be mailed to the address present on your drivers license 90 days before it’s due if you are already a holder of a class A, B, C, D, E, or F licence.

Assessment and form completion is $200.
